Introduction:


Sierra Group Home is a private owned home nestled in serine northern Wisconsin. Adjacent to the Wolf River the home is owned and operated by Steven and Michelle Dunn of White Lake Wisconsin. It is currently licensed for eight adolescent males, ages twelve to seventeen.

The program provides direction and guidance in a therapeutic environment. The goal of Sierra Group Home is to successfully return the adolescent to his community. It is our philosophy that this is accomplished by providing behavior modification in a positive environment.

A phone call or receipt of referral materials will begin the placement process. Steven and Michelle work closely with their sponsoring agency, Lutheran Social Services and Upper Michigan, Inc.

Particular emphasis is placed on education, personal accountability, along with successful reintegration into the community.

Education:


Clients attend White Lake High School whose staff works closely with the group home. Students that require special needs the school provides individualized educational planning through its LD/ED program. All clients will be monitored by implementation of daily school sheets and weekly conferences with school staff. Sierra encourages clients to participate in extra curricular activities.

Recreation:


Sierra Group Home's location offers a wide variety of recreation experiences such as: camping, fishing, canoeing, wall climbing, downhill, and cross country skiing. Team building and confidence initiatives are the focal point in this facet of treatment.

Admission Process:


All clients must be court ordered into placement. Referral material must include:

  • Current medical, psychological, and educational information
  • Social history
  • Legal history

Placement Material:


  • Social Security Card
  • Birth Certificate
  • Immunization Record
  • Medical and Dental History
  • Insurance Information
  • Other pertinent information

After a review of the referral a preplacement visit will be scheduled to discuss historical data and Sierra program with the client, parents, and social worker. Following the preplacement visit, the administrators will make a decision with the sponsoring agency on placement within three business days of receipt of referral.
